Informed Consent and Agreement Form
Purpose of Informed Consent Form The purpose of this informed consent form is to establish clear parameters between Mission Elite Performance Inc. ("Mission Elite") and you, the "Client/Athlete," for the purpose of mental performance coaching. By signing this document, you acknowledge and agree to its terms. If you have any questions or concerns regarding the form, please do not hesitate to ask.
Mental Performance Coaching Mental performance coaching is a resource for athletes, teams, and coaches to enhance their mental skills for improved performance. Coaching sessions may cover topics such as building confidence, improving focus, and increasing resilience. It is important to note that participation in mental performance training may occasionally lead to feelings of frustration or discomfort. Additionally, there is no guarantee that participation in mental skills training will result in success.
Mental Performance Coaches Background I, Raheel Manji, have extensive experience working with athletes and teams at various levels to support their mental performance development. My educational background includes studying topics such as the Psychology of Performance through the National Coaches Certification Program, Building High-Performing Teams through Stanford University, and Executive Leadership Coaching through Harvard University.
Client Autonomy Mental performance coaching is a collaborative process. You have the right to participate in shaping the direction, techniques, and goals of the coaching sessions. You may choose not to participate in specific aspects of the consulting process or terminate the mental performance coaching relationship at any time without penalty.
Confidentiality and Limitations Mission Elite is committed to maintaining the confidentiality of information shared during 1-on-1 sessions, subject to the following exceptions:

If there is an immediate danger to yourself or others, confidentiality must be breached.

Any instances of abuse involving a child, elderly person, or disabled individual must be reported.

If records are subpoenaed by a court of law, they must be provided.

Dependent adults’ guardians may impose limitations on confidentiality.

In the event of an unforeseen situation requiring a breach of confidentiality, every effort will be made to notify you beforehand.
For online sessions conducted via platforms such as Zoom, absolute confidentiality cannot be guaranteed due to potential security risks inherent to internet-based communications.
Class Sessions and Public Recording Information shared during group/class mental performance sessions is not confidential. Mission Elite reserves the right to record these sessions and use the recordings for informational, marketing, or media purposes, including but not limited to sharing on platforms such as YouTube and social media. By participating in these sessions, you consent to being recorded and understand that your likeness and contributions may be made publicly available.
Assumption of Risk and Waiver of Liability You acknowledge that mental performance coaching is a voluntary activity and understand the inherent risks, including but not limited to emotional discomfort and the lack of guaranteed results. Through this agreement, you release Mission Elite and its representatives from liability for any claims, damages, or losses arising out of or related to your participation in coaching sessions, to the fullest extent permitted by law.
Informed Consent I have read and understand the information provided in this document. I consent to participate in mental performance coaching and agree to the terms outlined above.
