Mission Elite Training Camp Player Contract

This document is a recognition of the agreement made between the purchaser of the program and/or participant of the program, and Mission Elite Performance. The purpose of this contractual agreement is to allow parents and players to give informed consent for a player to participate in the Mission Elite 2024 Tennis Camps or any Mission Elite Tennis Training Camps. By checking the service agreement box, you hereby agree to the following terms laid out in this contract by Mission Elite Performance. 

Section #1: Refund Policy

There will be no refund on the amount paid through the online registration apart from days/weeks missed, that is due to the following; 

  • Extreme Illness/Injury (that will be reviewed and judged by Mission Elite Executive Staff)
  • Personal Issues & Conflicts (that will be reviewed and judged by Mission Elite Executive Staff)

The refunds will only be applicable if Mission Elite performance is notified 14 days prior to the specified week of training starts.  Please note, that a 7.9% processing fee will be deducted from your refund.

Section #2: On-Court Injuries & Incidents

Mission Elite Performance coaches and support team will be there to supervise and take care of the players, however, Mission Elite Performance will not be held liable for any injuries and/or incidents that occur on the court during scheduled programming for any of the programs that are being run. 

Section #3: Behavior & Coaching Standards:

The purchaser of the program and/or participant of the program is required to meet and fulfill all expectations of the program, including the Mission Elite Values, Duties, Expectations, and Consequences. Any serious violations of these values and expectations will result in expulsion from the program. 

Expectations:

  • Mission Elite expects respect from the players and parents towards the coaching staff.
  • Mission Elite expects effort from the players at all times. 
  • Mission Elite does not tolerate offensive or inappropriate language on or off the court. 
  • Mission Elite does not tolerate aggressive and negative reactions and behaviors.  

 Section #4: Fitness to Participate

The purchaser of the program and/or participant of the program agrees and acknowledges that he/she is physically and mentally able to participate in the program and has no current or prior physical or mental conditions that would present a risk to other players, parents, and/or coaches.

Section #5: Medical Insurance

The purchaser of the program and/or participant of the program will assume responsibility for medical costs incurred as a result of illness and/or as a result of the purchaser of the program and/or participant of the program’s participation in the program. The purchaser of the program and/or participant of the program agrees to pay for any costs related to their medical treatment. 

Section #6: Damages

The purchaser of the program and/or participant of the program will assume responsibility for damages that he/she inflicts on the Tennis Club or Pickering Recreation Complex or Chestnut Hill Developments Recreation Complex premises.

Section #7: Photographs & Media

The purchaser of the program and/or participant of the program consent to the use by Mission Elite Performance for any photographs/media taken and distributed for publicity, advertising, promotions, and/or other professional purposes. 

Section #8: Equipment

The purchaser of the program and/or participant of the program will be expected to enter the program with his/her own equipment, except for tennis balls. 

Section #9: Agreement Outside the Contract

The purchaser of the program and/or participant of the program agrees that he/she has entered into this contract under their own volition, and is of sound mind and body.

I, the purchaser of the program and/or participant of the program, acknowledge that I have carefully read this agreement and fully understand its contents and meaning. I further acknowledge that this Agreement will absolve and release Mission Elite Performance from any liability in connection with any injury or harm suffered as a result of my/my player’s participation in the Mission Elite Training Academy. I acknowledge that I have been made aware of any and all risks of participation in the camp activities. 

Section #10: Registration

I, as the purchaser/participant, understand that my spot will not be secured until 7 days before the week of the program I register for. I understand that spot security in the camp will be based on the highest UTR’s that register. I understand that I will be refunded 7 days prior (or be moved to the next week if possible) if my spot is not secured and I am outside the range of UTR’s applicants that register for that week.

  • Location: Heron Park Tennis Club (292 Manse Rd, Scarborough, ON M1E 3V4

*Due to administrative demand, no refunds or adjustments for any reason will be offered. All openings will be offered to the highest UTR's first.
